The GP Toolkit is an information system which supports GP Practices and Primary Care
Trusts in working with their local hospitals to introduce alternative, and more effective
care, closer to home. It identifies opportunities to move money and resources around
the health system to improve services, both inside and outside of hospital.
This release of the GP Toolkit covers admitted care and based on the ‘best practice’
template identifies where services need to be improved - avoiding admissions, earlier
discharge from hospital and improved patient management.
Further releases of the GP Toolkit are planned which will include outpatient services,
public health indicators, ‘risk stratification’ and prescribing.
The information in the GPToolkit is periodically updated and you can set up an alert
email to notify you when data sets have been refreshed.
The SHA and Teamwork are planning regular ‘topic updates’ which look at an
area of best practice in more detail. Details of future ‘topics’ are listed on the ‘Notice
Board’.
The GPToolkit will also be available shortly to GP Practices throughout the new West
Midlands Strategic Health Authority.
